A preliminary examination of the blown jet engine of the Southwest Airlines plane that set off a terrifying chain of events in the USA and left a woman hanging half outside a shattered window showed evidence of "metal fatigue", according to the National Transportation Safety Board.

European regulators last month required airlines flying in Europe to conduct the inspection, but the FAA had not yet required them despite proposing a similar directive last August.
