Hidden Crack in Southwest Jet Engine at Center of NTSB Investigation

A preliminary examination of the blown jet engine of the Southwest Airlines plane that set off a terrifying chain of events in the USA and left a woman hanging half outside a shattered window showed evidence of "metal fatigue", according to the National Transportation Safety Board.

European regulators last month required airlines flying in Europe to conduct the inspection, but the FAA had not yet required them despite proposing a similar directive last August.

A team from the National Transportation Safety Board reports that a preliminary inspection shows a metal fan blade failed, the same thing that happened two years ago.

But engine maintenance and inspection protocols vary by airline, Goglia said, adding that airlines, in consultation with engine manufacturers, must get their maintenance protocols approved by the Federal Aviation Administration. EASA said the directive was issued after the failure of a fan blade on a CFM56 engine, which led to the uncontained release of debris.

Sumwalt expressed concern about such a destructive engine failure but said he would not yet draw broad conclusions about the safety of CFM56 engines or the entire fleet of Boeing 737s, the most popular airliner ever built.

They were developed by French-US joint venture CFM International, which says it is the "world's leading supplier of jet engines for single-aisle aircraft". On Tuesday, the Dallas-bound Southwest flight 1380 was 32,500 feet over Pennsylvania when the plane's engine fan blade broke off, causing debris to fly off. NTSB officials have told stories about absent parties who were blamed in a probable cause finding because they were absent from the investigation, and it was politically easy to assign blame to an absent party. The airline said it was in the process of transporting them to the terminal and was gathering more information about the incident.

One person was killed, and seven were hospitalized with minor injuries, authorities said.

The very idea that a passenger could be sucked out of an airplane mid-flight has rattled many travelers. "There was a lot of blood because she was hit by some of the shrapnel coming off the engine after it exploded".

Inside the cockpit, pilot Tammie Jo Shults calmly communicated the severity of the situation.

Many social media users defended Martinez's use of Facebook Live, but some said he violated passengers' privacy and sought cheap fame. Needum and retired school nurse Peggy Phillips began administering CPR for about 20 minutes, until the plane landed. "Our hearts are heavy", the two pilots said in the statement.

Whatever happened inside that complicated system of fuel and fire and whirling components, it resulted in the decompression of the cabin at about 32,000 feet and, later, the death of a passenger after she almost flew out a window. The oxygen masks came down, and he first made sure his family was secure. But during the descent, Martinez said he and some of the plane's other passengers thought they wouldn't land safely.

She tells traffic control, "Yeah, we have part of the aircraft missing so we're going to need to slow down a bit". Southwest flight from NYC to Dallas!' He said he noted that a woman behind his wife was struggling to get an oxygen mask on her 6-month-old son and went to help. "OK, and is your plane physically on fire?"
